What to Bring to Your Appointment
Having these ready will help us provide the best care for you
Medicare Card
Required for bulk billing. Please ensure your Medicare card is current and valid.
Current Medications
Bring a list of all current medications, vitamins and supplements you are taking.
Referral Letters
Any referral letters, previous test results or specialist reports relevant to your visit.
Photo ID
A valid form of photo identification such as a driver's licence or passport.
Emergency Contact
Details of a family member or emergency contact in case they are needed.
Health Fund Card
If applicable, bring your private health insurance card for allied health services.
Cancellation Policy
⚠️ Please give at least 2 hours notice if you cannot attend your appointment.
A $40 Did Not Attend (DNA) fee applies if you miss your appointment without adequate notice. This allows us to offer your appointment time to another patient in need.
